Questions
What size should my source image be?
Square and at least 512×512. Anything smaller will look soft at the larger sizes, and non-square images are center-cropped to fit.
Do I still need a .ico file?
Not for any browser released this decade — PNG favicons are universally supported. A .ico is only worth it if you must support Internet Explorer.
Why does my favicon look like mush at 16 pixels?
Because detailed logos do not survive being shrunk to 16×16. Design a simplified mark for small sizes: one shape, high contrast, no text.
